| Release Waiver
In consideration of registering to participate in Gay Games VII, and for other good and valuable consideration, receipt of which is hereby acknowledged, I hereby grant to: Chicago Games Inc.; Federation of Gay Games, Inc.; and their respective subsidiaries, affiliates, successors, assigns, and to such other persons as they may designate from time to time, (collectively, “Licensees”), the absolute right and permission to use, in perpetuity, my name, likeness, voice and photograph(s) of me, either alone, or accompanied by other persons or material, in any manner and in any media, throughout the world, at any time hereafter, for any lawful purpose.
I hereby waive my right to inspect and approve any photograph(s) and/or other material(s), the use, or such written or spoken version of which, as may be used in connection therewith. I agree that any picture(s) taken of me by Licensees, are owned by Licensees. I understand that nothing herein will constitute any obligation on the part of Licensees to make any use of all or any part of my name, likeness, voice or photograph.
I understand and agree that if I should receive any print, negative or other copy thereof, I shall not authorize its use by anyone else.
I agree that I will not hold Licensees, or anyone who receives permission from them, responsible for any liability resulting from the use of my name, likeness, voice or photograph(s) in accordance with the terms hereof, including what I might deem to be a misrepresentation of me, my character or my person, due to distortion, optical illusion or faulty reproduction which may occur.
I warrant and represent that I am fully familiar with, the gymnastic, sporting or other equipment which I will use during Gay Games VII, that I have often, prior to this time, performed such gymnastic, sporting or other activities similar to those which I will perform in Gay Games VII. I acknowledge that I have a complete understanding of the risks involved in performing them. I make these representations to you with the full understanding that you are relying upon them.
I hereby accept all risks attendant thereto and thereby agree to indemnify, release and hold Licensees harmless from any liability and/or injury that I might suffer, during or due to my gymnastic, sporting or other performance. I understand that my name, biographical information and/or my photograph and/or likeness may be provided to the Federation of Gay Games, Inc., and I consent and agree to such provision of this information.
This Release shall be governed by the laws of the State of Illinois and of the United States of America, and actions arising hereunder or relating here to shall be brought in a court of competent jurisdiction in the City of Chicago, State of Illinois, the United States of America. I am over 18 years of age and have the authority and the right to enter into this Agreement and to be bound by the terms thereof.
